Brief Description of Duties:
University Hospital of Brooklyn is creating an Emergency Deparmtent Care Management (EDCM) function through the establishment of an ED Triage Transitional Team, to be comprised of a RN Care Manager, a Social Worker and a Community Liaison Worker. The role of the team is to visit referred patients in the hospital for the initial assessment; coordinate with other care management services available to the patient; provide self-management support; establish linkages with community based resources; work with patients on obtaining medications, DME and supplies; and communicate with discharged patients via telephone or in person during the post discharge period. The Community Liaison Worker will work alongside the Social Worker in carrying out home visits to follow up with patients, ensuring patient self-management, obtaining prescriptions, engaging in healthy living and other activities to obtain am maintain a healthy status. Patient population to be addressed is based on the following criteria: 1. Prior hospitalization or ED visits in last 90 days; 2. Risk of medical decompensation on absence of close short-term follow-up; 3. Behavioral Health condition that interferes with self-care; 4. Insufficient social support to meet follow-up needs, or high caregiver stress; or 5. Significant recent declline in activities of daily living (ADL). The Community Liaison Worker will work collaboratively with all levels of the ED staff and provide administrative support to the EDCM team. Coordinate with other care management services available to the patient linkage to primary care and other outpatient care providers.

|All successful candidates must undergo various background checks, maintain credentials required for continued employment and adhere to the SUNY-DMC UHB Principles of Behavior. Clinical Faculty and Allied Health professionals must receive and maintain Medical Board authorization.|
REQUIRED: Bachelor's degree in healthcare related fields. NYS Driver's License (car preferred but not a must). Minimum of one year's experience in acute or long-term care with prior Medicare/Medicaid and uninsured guidelines. Experience with home visit. Knowledge of case management.
